Abstract

Background: Psoriasis is a chronic immune-mediated skin disease driven by IL-23/Th17 axis. TSLP, an epithelial-derived alarmin cytokine, has emerged as an upstream regulator of psoriasis, TSLP rs1837253 has been proposed as a functional variant influencing TSLP secretion, although its role in psoriasis remains insufficiently characterized. Aim: This study aimed to evaluate serum TSLP levels in relation to disease severity (PASI) and to determine the genotypic and allelic distribution of the TSLP rs1837253 polymorphism in psoriasis patients compared with controls. Methods: Sixty patients with psoriasis and 60 age- and sex-matched healthy controls were recruited. Serum TSLP was quantified by ELISA, and rs1837253 genotyping was performed using Tetra-ARMS-PCR. Results: TSLP was significantly elevated in patients vs controls (median 1812.08 vs 523.52 pg/mL, P = 0.001) but did not differ significantly among disease subgroups (P = 0.833). ROC analysis identified a cutoff of 736.06 pg/mL yielding good diagnostic accuracy (AUC = 0.819, sensitivity 93.3%, specificity 73.3%). The control's rs1837253 genotype distribution conformed to HWE (P = 0.229). Genotype frequencies did not differ significantly between groups (P = 0.067), but the C allele was significantly more frequent among patients than controls (58.3% vs 44.2%; P = 0.023). TSLP levels did not vary significantly by rs1837253 genotype. Conclusion: TSLP is elevated in psoriasis and performs well as a discriminatory biomarker, independent of clinical severity. rs1837253 C allele appears to confer genetic susceptibility to psoriasis, The T allele behaves as a protective variant, supporting a role for TSLP-related genetic variation in disease predisposition.
